Инфекционный эндокардит (ИЭ), вызванный микроорганизмами Burkholderia cepacia — очень редкая и недостаточно охарактеризованная форма эндокардита.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Мы наблюдали случай позднего ИЭ протезированного митрального клапана вызванный Burkholderia cepacia у пациентки 34 лет.</p></sec><sec><title>Результаты</title><p>Результаты. Пациентка с врожденным дефектом межжелудочковой перегородки перенесла три операции на сердце, в том числе удаление вегетаций на митральном клапане в связи с ИЭ в 17 лет и протезирование митрального клапана с повторной пластикой ДМЖП в 33 года. Последняя операция осложнилась гнойным стерномедиастенитом, а через 10 месяцев после торакомиопластики у больной появилась лихорадка с ознобами, была выявлена крупная вегетация на протезе митрального клапана и при бактериологическом исследовании крови получен рост Burkholderia cepacia с мультирезистентностью к антибиотикам. После начала лечения триметопримом/сульфаметоксазолом наблюдалась нормализация температуры, однако течение ИЭ осложнилось тромбоэмболиями с летальным исходом.</p></sec><sec><title>Выводы</title><p>Выводы. Мультирезистентность возбудителя к антибиотикам, в том числе тем, которые эмпирически назначаются при ИЭ — основной фактор риска неблагоприятного исхода ИЭ, вызванного Burkholderia cepacia. Трудности лечения ИЭ, вызванного Burkholderia cepacia усугубляет отсутствие общепризнанных рекомендаций, определяющих дозы антибиотиков, которые назначаются в соответствии с чувствительностью выделенного возбудителя. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Infectious endocarditis (IE) caused by microorganisms Burkholderia cepacia is a very rare and poorly characterized form of endocarditis.</p></sec><sec><title>Material and methods</title><p>Material and methods. We observed a case of late prosthetic mitral valve IE caused by Burkholderia cepacia in a 34-year-old patient.</p></sec><sec><title>Results</title><p>Results. A patient with a congenital ventricular septum defect underwent surgery on the heart three times in past, including the resection of mitral valve vegetations due to IE at age 17 and mitral valve replacement plus ventricular septum defect plastic reconstruction at 33 years old. The last was complicated by postoperative purulent sterno-mediastenitis treated by thoracoplasty. 10 months later the fever with chills appeared again, a large vegetation on a mitral valve prosthesis was revealed, and Burkholderia cepacia bacteremia with multidrug resistance to antibiotics was find. After the start of treatment with trimethoprim / sulfamethoxazole, normal temperature was observed, but the course of IE was complicated by thromboembolism with a fatal outcome.</p></sec><sec><title>Conclusions</title><p>Conclusions. Multidrug resistance of the pathogen to antibiotics, including those empirically prescribed for IE, is the main risk factor for the adverse outcome of IE caused by Burkholderia cepacia.
