Метаболический синдром (синонимы: синдром X, синдром инсулинорезистентности, кардиометаболический синдром) характеризуется увеличением массы висцерального жира, снижением чувствительности периферических тканей к инсулину (инсулинорезистентностью) и гиперинсулинемией, которые вызывают нарушения углеводного, липидного, пуринового обменов. Артериальная гипертония является неотъемлемым компонентом метаболического синдрома. Тяжесть артериальной гипертонии у больных с метаболическим синдромом выше по сравнению с пациентами без метаболических нарушений. У больных с метаболическим синдромом вероятность поражения сердца и мозга увеличивается в 5 раз, почек — в 3 раза, сосудов — в 2 раза. Наличие сахарного диабета снижает вероятность достижения эффективного контроля артериального давления в 1,4 раза, гиперхолестеринемии — в 1,5 раза, ожирения — в 1,7 раз. При наличии любых 3 факторов эффективность лечения снижается в 2 раза. В статье с позиций современных рекомендаций представлены подходы к ведению пациентов с артериальной гипертонией и метаболическим синдромом, особенности немедикаментозной терапии, целевые уровни артериального давления, выбор препаратов в соответствии с данными доказательной медицины.</p></abstract><trans-abstract xml:lang="en"><p>Hypertension is one of the key risk factors for cardiovascular morbidity and mortality. Metabolic syndrome (synonyms: syndrome X, insulin resistance syndrome) is characterized by increased visceral fat mass, decreased sensitivity of peripheral tissues to insulin (insulin resistance) and hyperinsulinemia, which cause disorders of carbohydrate, lipid, and purine metabolism. Hypertension is an integral component of the metabolic syndrome. The severity of hypertension in patients with metabolic syndrome is higher in comparison with patients without metabolic disorders. In patients with metabolic syndrome, the probability of cardiac and brain damage increases fivefold, kidney damage threefold, and the vessels twofold. The presence of diabetes reduces the likelihood of achieving effective control of blood pressure by 1.4 times, hypercholesterolemia — by 1.5 times, obesity — by 1.7 times. In the presence of any three factors, the effectiveness of treatment is reduced twofold.
