The problem of osteoporosis treatment is very urgent for professionals around the world, but in different countries different strategies are adopted to address this issue. This article presents a comparison of domestic and European approach to the diagnosis and treatment of osteoporosis. Differences in the organization of the process of diagnosis and treatment are available. But on some issues, opinions of experts agree, particularly in Russia and in Europe bisphosphonates recognized as “gold standard” of treatment of osteoporosis. The panelists recognize Foroza® as affordable medicine with high efficacy and safety.
РАЦИОНАЛЬНАЯ ФАРМАКОТЕРАПИЯ
Systemic enzyme therapy (SET), developed by M. Wolf and K. Ransberger in 1954, is a method of treatment and prevention of diseases with the help of specially selected combination of hydrolytic enzymes of vegetable or animal origin (biological catalysts) that affect key physiological and pathophysiological processes in body. Systemic effects of exogenous enzymes provided direct or indirect effect on the hydrolase activity of the major organs and body systems through the modulation of their own enzyme systems or system antiproteaz and cytokines.
In the article approaches to treatment of patients with a nonspecific back pain are considered. Aspects of application Vitamins B (Milgamma) in complex therapy of patients with painful syndromes caused by inflammatory and degenerate defeats of a backbone are shined. It is noticed that the Vitamin B neurotropic complex can enhance analgesic effect of nonsteroidal anti-inflammatory drugs, allowing to make lower a dose and administration of these drugs, and also favours faster recourse of a painful syndrome.
Treatment of patients with infectious mononucleosis is currently difficult because none of the known antiviral drugs can completely eliminate the virus from the organism. Promising is the use of genetically engineered recombinant α-interferon (Viferon®) with a complex antiviral, antibacterial, antiproliferative, and immunomodulatory effects. In our study Viferon® showed high efficiency and was well-tolerated by patients. Viferon® can be recommended for complex treatment of EBV-infected children.
ГАСТРОЭНТЕРОЛОГИЯ
The authors present their own clinical experience in treating elderly patients with gastroesophageal reflux disease using different therapy regimens: monotherapy with proton pump inhibitors (Omeprazole Sandoz) and H2-histamine blockers using a combination of a prokinetic in different modes of observation. It is shown that use of Omeprazole Sandoz is preferable in terms of efficacy, safety and cost of treatment.
Helicobacter pylori (HP) eradication therapy is a compulsory component of the etiological treatment of HP-associated diseases, but modern standardized schemes of eradication therapy do not lead to complete destruction of HP. Determination of the phenotype of the oxidizing metabolism allows to personalize the dosage of the drugs in the treatment of the patients with peptic ulcer. This approach allows to reach maximum efficiency and eradicate HP, thus, making essential impact on the forecast of the disease and quality of life of the patients.

The article summarizes facilities of moxonidine (Physiotens®) — currently one of the most commonly used antihypertensive medicine from sympatholytics. In addition to effects on blood pressure, we provide data on the effects of moxonidine (Physiotens®) on the metabolism, glucose tolerance; studies show that it is a medicine of choice for treating hypertension in post-menopausal women. It is also estimated that moxonidine (Physiotens®) is advantageous from the viewpoint of pharmacoeconomics.
Nicorandil is a dual pharmacological mechanism anti-anginal agent with adenosinetriphosphat-esensitive potassium (Кatp) channel agonist and nitrate-like properties. The aim of this review is to provide an update on the efficacy of treatment with nicorandil. First, the rationale for nicorandil treatment and its pharmacological effects are reviewed. We explain the mechanisms underlying ischemic preconditioning, with a focus on those that involve the KAТР. Next, clinical practice related lo ischemic preconditioning and pharmacological preconditioning with nicorandil as well as other favourable mechanisms of improvement of prognosis, in which it plays a role in improving endothelial function, modulating autonomic nervous system activity and stabilizing plaque are summarized. Finally, the tolerability of nicorandil is discussed.
ГЕМАТОЛОГИЯ
The authors present their experience of clinical observation and treatment of 42 patients with chronic heart failure (CHF) in combination with irondeficiency anemia (IDA) using iron supplements for parenteral (iron karboksimaltozat — 7 patients) and oral (Ferlatum — 42 patients) use. The role of IDAin the formation of the severity of CHF functional class was studied. It is shown that iron supplementation are clinically effective in treatment of chronic heart failure: with an increase in serum iron at 97.7% level of the hemoglobin (Hb) increased by 15.4%, left ventricle ejection fraction — by 32.2%, which allowed almost in 100% to reduce functional class of CHF. Inclusion of Ferlatum in the treatment program for CHF provides clinically necessary recovery in Hb (110–120 g/l) at IDA with minimum side effects.
Anemia is the most common blood disorder. There are several kinds of anemia, produced by a variety of underlying causes. Anemia can be classified in a variety of ways, based on the morphology of red blood cells and underlying etiologic mechanisms. Anemia goes undetermined in many people, and symptoms can be minor or vague. The signs and symptoms can be related to the anemia itself, or the underlying causes. This article is about the main principles of diagnosis of anemia in clinical practice. We hope that this information will help to diagnose the different types of anemia in daily clinical practice.
СИМПТОМАТОЛОГИЯ И ДИАГНОСТИКА
We examined 98 patients with chronic obstructive pulmonary disease (COPD) of varying severity in active phase of the fading. Levels of IL-8, γ-IF, TNF-α in sputum, bronchial washings and serum of patients with COPD were studied. Correlation between cytokine data with cytological structure of sputum and bronchial washings and C-reactive protein serum depending on the stage of disease was confirmed. Communication of smoking with activity of local and system inflammatory process was revealed.

This article shows the special features of the influence of chemotherapy on the indices of the activity of ribosomal genes in vivo. Reduction in the indices of the functional activity of ribosomal genes on 10 chromosomes after the first course of poly-chemotherapy and increase after the third course is discovered. It is shown that this modification effect appeared due to the indices of the functional activity of ribosomal genes on the D-chromosomes. Index on the G-chromosomes demonstrated its stability. Also an increase in the number of chromosomes was shown, which entered association, and number of associations of acrocentric chromosomes. The observed differences in the indices of the activity of ribosomal genes with different courses of polychemotherapy reflect the special features of response reactions to the manifestation of exogenous actions of chemical ingredients.
This article presents the results of a study of the protein composition of erythrocyte cell membrane and functional activity of ribosomal genes in 85 patients with chronic bronchitis of nonprofessional genesis and 68 healthy persons. Found that in erythrocytes of patients has been quantitative imbalance of the number of cytosceletal, transport proteins, and proteins responsible for the mechanical properties of erythrocyte cell membrane. Identified either a reduction, or increase of functional activity of ribosomal genes in patients, compared with a control group. Shown thаt changing the quantitative characteristics of molecular composition of erythrocyte membranes affects indicators of cytogenetic variability in patients with chronic bronchitis of unprofessional etiology. In view of the biochemical, cytological and clinical aspects of pathology developed model, allowing 91% accuracy to predict the development of chronic bronchitis.
