Abstract

According to WHO interpretation diabetic polyneuropathy, or chronic sensorimotor polyneuropathy, distal symmetrical polyneuropathy is a condition characterized by progressive loss of nerve fibers, leading to loss of sensation and the development of foot ulcers in diabetes mellitus. Almost 40% of diabetic patients revealed the phenomenon of the autonomic nervous system disorders that can lead to myocardial infarction, including silent forms. Effective treatment of these conditions and the prevention of complications provide Milgamma, Milgamma compositum and Thiogamma, as confirm from the standpoint of evidence-based medicine.

Abstract

The relevance of cardiovascular disease in the world and in Russia causes including calls for patient compliance with medical recommendations. Compliance, in turn, depends on the cost of medicines. From an economic point of view generic drugs appear in a very favorable light, price is several times lower than the original’s. However, comparative studies of therapeutic equivalence of generic and branded drugs are very few. This paper presents a comparative clinical evaluation of the original drug atorvastatin and its analogue (Torvakard (Zentiva)), the lipid-lowering and endothelium-protective effects of both drugs were studied.

Abstract

The article presents data on the characteristics of flu-like A(H1N1) pdm09 pneumonia in patients of pulmonology department according to clinical, laboratory and instrumental methods. The sexual and age structures of patients have been studied. It was shown the significance of the epidemiological situation. Antiviral and antibacterial chemotherapy of influenza pneumonia have been analyzed.

Abstract

Treatment of acute respiratory viral infections (ARVI) and influenza remains one of the most urgent problems of modern medicine. Selection of drugs should take place taking into account the criterion of efficacy, safety, ease of use and accessibility to the patient. In this article the domestic immunomodulator, and cytoprotector reparant Derinat natural origin. The authors concluded that Derinat best meets these requirements and can be  recommended as drug of choice for pediatricians and physicians. For the treatment of uncomplicated cases of influenza and ARVI Derinat 0.25% is recommended and in case of severe “cold” and complicated forms of viral infections — Derinat solution of 15 mg/ml for parenteral administration.

Abstract

The article presents current data on the prevalence, modern methods of diagnosis and treatment of mixed tick-borne neuroinfections. Particular attention is given to a mixed tick-borne encephalitis — borreliosis. The authors provide a detailed description of the clinical picture of disease, lead-effective diagnostic algorithms and the latest treatment recommendations.

Abstract

Timely and accurate diagnosis and early start of the treatment can improve the prognosis and clinical course of acute coronary syndrome. In some cases (in the absence of persistent ST-segment elevation, non-specific ECG changes, bundle brunch blocks, atypical presentations of the disease) differential diagnosis between acute myocardial infarction and unstable angina is difficult. The currently applied markers of myocardial necrosis — heart troponins and CK-MB are not effective enough in the early stages of AMI. On results of the multicentre study ISPOLIN, which included 1049 patients in 17 cities of the Russian Federation, the new domestic express-test for the qualitative determination of cardiac fatty acid binding protein, has more sensitivity, accuracy and predictive value compared with the test of the troponin I in the first 12 h of AMI and can be recommended for wide application in practice.

Abstract

428 practically healthy individuals and 301 patients with arterial hypertension have been examined by using a volumetric sphygmograph VaSera-1000 (Japan). In young individuals elevated stiffness of elastic arteries is associated with elevation of predominantly systolic blood pressure (BP) and elevated stiffness of muscular arteries is associated with elevation of diastolic BP. CAVI stiffness index > 7,6, ankle-brachial PWV > 11,9 m/s and heart-brachial PWV > 6,4 m/s are the criteria of early vascular aging in young people. Early remodeling of elastic arteries was observed in 47,1% patients with isolated systolic hypertension, in 31% patients with systole-diastolic hypertension and in 12,3% healthy subjects. Early remodeling of predominantly muscular type of arteries was observed in 54,4% patients with systole-diastolic hypertension, in 15,5% patients with isolated systolic hypertension and in 14,5% practically healthy subjects.

Abstract

We give a case report of idiopathic pulmonary arterial hypertension (PAH) in a male patient with cancer of kidney. The hypothesis of pathogenetic link between these diseases was made. The cause of pulmonary hypertension could be cancer with ectopic production of biologic active substances, growth factors in combination with the susceptibility to develop PAH in poor gene penetrance.

Abstract

The chronic disease of kidneys represents a serious problem of the world health service. Last years there has been an important increase of patients with chronic kidney disease, literature revealed new information about character lesions of the cardiovascular system at patients with chronic kidney disease. This article shows that the combination of chronic kidney disease with cardiovascular pathology makes worse the prognosis of life. In this context, a special role plays an exploring the possibilities of timely correction of the treatment of these diseases.

Abstract

Clinical geriatric practice, you must exclude the idea of old age as a disease. An active lifestyle in later age depends on many factors, understanding that successful implementation of policies and programs aimed at creating an integrated model. Relevant in this regard is the need to analyze the basic medical and social factors, narrowing the possibility of extending the active lifestyles of elderly people. Each of the stages of medical and social assistance to the elderly and senile age has its own characteristics to be taken into account in the further improvement of this category of persons. And only their harmonious combination will improve quality and save the active life position for many years.

Abstract

Type 2 diabetes is one of the most important social diseases. Improving the quality of life for patients with diabetes is one of the expected outcomes of the implementation of programs designed to optimize the organization of diabetes care to patients with type 2 diabetes mellitus. Evaluation of its effectiveness is carried out based on population and quality of life. Quality of life in patients is largely dependent on the patient’s place of residence (urban or rural). This article presents data on the quality of life of patients with type 2 diabetes according to sex and age living in rural areas.

Abstract

The factors supposed to influence on the quality of life in patients with bronchial asthma and arterial hypertension had been examined. MOS SF-36 v2 questionnaire was used. Age, sex, country life, association of the professional occupation with stress had been contributed to decrease in quality of life. Bronchial obstruction and long asthma history correlated with worse parameters of SF-36 v2. Arterial hypotension lower than 120 mm Hg against the background of antihypertensive therapy correlated with decrease in quality of life.

Abstract

In the North Caucasus Federal District operate seven scientific medical societies of physicians as an autonomous. Societies have a different history and experience. Thus, the association of therapists in Dagestan, North Ossetia and the Stavropol region exist during many decades. In other regions of the Northern Caucasus societies are relatively young due to the peculiarities of formation of the federal entities. Almost all regions have on their territory higher medical educational institutions — academies and medical faculties of universities, providing a scientific, methodological framework and a high level of clinical therapeutic services.
