Abstract

According to big clinical trails Mg 2+ deficiency plays a major role in health and promote development of different chronic diseases. It is necessary to cooperate clinical symptoms and results of objective methods (ECG, myography, bone densitometry, level of Mg 2+ in hole blood, erythrocytes, serum, saline, urine, nails, hair) in case of diagnosis Mg 2+ deficiency. It is desirable to analyze Mg2+ intake with food by standard questionnaires. To have the full picture of Mg 2+ deficiency it is necessary to define level of Mg2+ in different biosubstrates.

Abstract

Chronic cerebrovascular attack (CCVA) is a brain lesion caused by vascular factors. CCVA appears as cognitive impairments (CI), affective (emotional) disorders and focal syndromes. Treatment for CCVA requires a comprehensive approach. Combination effective therapy for CCVA involves secondary prevention of stroke and CI; treatment of CI; treatment of depression and other affective disorders; neuroprotective therapy. Basic therapy for CCVA includes modification of risk factors, antihypertensive, hypolipidemic, and antithrombotic therapies. Central acetylcholinesterase inhibitors (galantamine, rivastigmine, donepezil) and a reversible NMDA receptor blocker (memantine) are symptomatically used at a stage of vascular and mixed dementia. There are no unique guidelines for the therapy of mild and moderate vascular nondementia-related CI. Drug use, based on the neurochemical mechanisms underlying the development of vascular CI, is substantiated. When choosing psychotropic agents, it is necessary to take into account the causes and clinical manifestations of neuromediator deficiency. Antidepressants are used as essential drugs. Neuroleptics and tranquilizers are additionally administered in complex-pattern syndromes, such as depression with marked anxiety. Prescription of neuroprotectors may be effective in treating both stroke and CCVA. These medicaments are most effective when a damaging factor acts, i.e. neuroprotectors should be given in a risk situation and to reduce damage. Citicoline is one of the most test drugs in a group of neuroprotectors.

Abstract

The article reviews the state of vascular-platelet hemostasis in patients with coronary heart disease and diabetes mellitus type 2. It was shown the less efficiency and the higher incidence of resistance to the classic double antiplatelet therapy in these patients after coronary stenting.

Abstract

In article we provided description of rare form of autoimmune pancreatitis. This rare form characterized by specific morphology picture, increase level of Ig, common comorbidity with other autoimmune diseases, positive result of corticosteroid therapy. This clinical case shows a situation with unapproved surgery treatment of a young woman that leads to disability and severe complications. Article includes short historical part about abundance, specific of this disease, classification and modern ways of treatment. We pay additional attention to differential diagnosis between autoimmune pancreatitis and cancer before perform any treatment especially surgery.

Abstract

The blood levels of molecules of immunoglobulins and selectins families in interrelation with the prognosis of liver cirrhosis were studied. The raised risk of death and low survival rate of patients within 1 and 3 years of observation were interconnected with high levels of ICAM-1, VCAM-1, E-selectin and P-selectin in blood. The revealed quantitative values of adhesion molecules which have prognostic importance concerning unfavorable events of liver cirrhosis can be used for estimation of survival rate of patients.

Abstract

To determine the prevalence of fatty liver as in patients with diabetes and implemented risk of chronic hyperglycemia: ischaemic heart disease and stroke. Methods: 1125 people, men — 552 women — 573 with ischaemic heart disease or stroke. Diabetes mellitus — 194, intermediate hyperglycaemia — 87. The age of patients from 54 to 92 years, mean age 69 ± 10.1 years. Results: the differences in the incidence of NAFLD in patients with diabetes and IGT — not found. Acute CVD in patients with both diabetes and with IG recorded during ultrasonic of the liver without NAFLD. Conclusion: the detection rate of NAFLD patients with DM and acute CVD ––13.9%.

Abstract

One of the most characteristic manifestations of rheumatoid arthritis is a chronic pain syndrome. Its features and the relationship with the presence of anxiety and depressive disorders insufficiently studied. The study found that patients with rheumatoid arthritis is characterized by high intensity of pain , which related with the assessment of the patient general health and is not significantly dependent on objective indicators of rheumatoid arthritis activity. Pain severity, anxiety and depressive disorders increased with functional class and radiological stage. There were significant correlations between the class of chronic pain by Van-Korff and the level of depression.

Abstract

In 24 patients with type 1 and 2 diabetes and 12 donors was shown that in diabetes the Nox releasing of membrane of blood formations is significantly enhanced. This phenomenon is more pronounced in type 2 diabetes, indicating a greater violation of the stability of erythrocyte membranes (EM) and serum exosomes (SE). Under the influence of exogenous glucose releasing of Nox is further enhanced to a greater extent, in particular from SE. Influenced insulin in vitro releasing Nox in EM and SE also increases in. Based on a number of data , it has been suggested the possible role of Nox as the insulin receptor. It is shown that in the serum membranous structures of patients with type 1 diabetes treated with insulin, as well as the addition of insulin into the blood of patients with type 2 diabetes the releasing of Nox was significantly higher than in healthy donors. In the presence of Embryonal antitumor modulator of Mkrtchyan (EATM) the releasing of Nox is significantly inhibited, especially in patients with type 2 diabetes. Suppression of the Nox isoforms releasing of EM and SE in patients with diabetes under the influence EATM can be considered as a possible mechanism of its membrane stabilizing effect in vivo.

Abstract

Nowadays lipohypertrophy remains severe insulinotherapy complication. Primary prevention is necessary for diabetic patients with pathologic areas of subcutaneous fat. Therefore, we developed the estimation risk model with good quality and high predictive value for diabetic patients who are under the treatment with insulin.

Abstract

Medical-economical standards are the analog of the classifiers of the diagnostically related groups (DRG). They are used as refunds to the hospitals from the insurance companies. The first to develop the DRG was Robert Fetter, the professor at the Yale University in the USA, in 1983, and used them to reduce increasing expenditures on the public health system (programs Medicare and Medicaid). In Russian system of public health the DRG could be used as means of determination of the costs of treatment in hospitals.

The analysis of changes in indicators ambulatory blood pressure monitoring (ABPM) in 26 patients with somatotropinoma. In 2/3 patients recorded systolic and diastolic hypertension. When analyzing their circadian blood pressure (BP) drew the attention of the lack of blood pressure reduction at night.

Abstract

We have fulfilled researches of frequency of atherosclerosis development in different arteries: brachycephalic vessels, abdominal aortic branch among 66 patients at the age of 45–60 with undifferentiated form of dysplasia of connective tissue (UDCT) compared with the group of 31 patients without dysplasia. We have revealed risk factors which influence extremely on origin and development of atherosclerosis: smoking, age, arterial hypertension grade III, left ventricular hypertrophy, underweight body.

Abstract

In recent years in cardiology actively studied the problem of breathing during sleep in connection with the fact that accumulate more and more data on the value of breathing during sleep in the development of cardiovascular disease, including fatal complications. In particular, great attention is paid to obstructive sleep apnea (OSA), as the condition is most clearly associated with cardiovascular disease. The prevalence of OSAS varies greatly in different study populations, and epidemiological data for Russia are practically absent. As the mechanism responsible for the increase in blood pressure in patients with OSA is usually considered activation of the sympathetic nervous system due to episodes of sleep apnoea. This pathophysiological process is also an important factor in the development of arterial hypertension in obesity. The results of researches, studying the mechanisms of interaction between obesity and OSA, very mosaic. Since OSA sleep much upset and is accompanied by episodes of hypoxia, there is nothing surprising that the secretion of hormones, usually having a circadian daily fluctuations, is also changed. These problems and devoted presented in this article the literature review.

Abstract

The aim of the study was the literature review of data about structural and functional changes of the myocardium in patients with different degrees of obesity, possible pathogenetic mechanisms of these changes and their dynamics in weight reduction. Some authors found normal geometry of left ventricular in people with obesity, whereas others got eccentric or concentric hypertrophy in the most of patients. The majority of the authors diagnosed normal systolic function and subclinical diastolic function. Absolute and indexed myocardium mass of left ventricular decreased after weight loss, positive correlations between their dynamics and anthropometric parameters were observed.

Abstract

In the article the author makes an attempt to reconstruct the history of the disease of the Russian poet A.A. Block on the basis of correspondence, memoirs of contemporaries, doctors who watched the poet. Medical issues diseases of the poet are discussed taking into account the historical situation, which has had some extent, the impact on the tactics of the patient.
