Abstract

Article is devoted to the role of body overweight in development of non-alcoholic steatohepatitis. Authors provide information about adipokines, peroxisome proliferator-activated receptors, proinflammatory cytokines in pathogenesis of nonalcoholic steatohepatitis. Authors analyze and describe special features of clinical picture and diagnostics, and also prophylaxis and treatment ways (changes in lifestyle, decrease of weight, possibility of metformin, thiazolidinedione, statins, essential phospholipids, vitamin E, ursodeoxycholic acid and probiotics treatment).

Abstract

A significant part of liver disease, once considered the result of infection by hepatitis B virus (HBV), are the result of infection HBV, and hepatitis delta virus (HDV). HDV is hepatotropic, defective RNA virus that requires obligatory HBV presence for its replication and the formation of infectious particles. About 10% of patients with chronic HBsAg-positive hepatitis infected by HDV. HDV results in coinfection (simultaneous infection of HBV and HDV) and superinfection (HDV infection with chronic hepatitis B). The natural course of the infection is characterized by rapid progression to liver cirrhosis and hepatocellular carcinoma. Diagnosis of HDV-infection is based on the detection of delta antigen, antibodies to the virus (anti-HDV IgM and IgG) and RNA HDV in serum. The effectiveness of antiviral therapy proved only on the treatment by interferon (IFN-2α and PEG-IFN-2α). The only way to protect against HDV infection is vaccination against hepatitis B.

Traumatic encephalopathy is an actual medical and social problem. The article describes the pathogenesis, clinical and therapeutic aspects of disorders in post-traumatic encephalopathy. Of particular interest from these neuroprotective for correction of neurological disorders is citicoline (Ceraxon), whose effectiveness is proven by numerous clinical studies and publications.

Abstract

Now in an arsenal of applied medicine the worthy place is occupied with preparations of interferons. And it is no wonder. Interferons as natural factors of nonspecific protection of an organism and mediators of immunity possess the widest spectrum of action. Interferons represent family of cytokines of the mediators playing the important role in formation of immunity and possessing expressed antiviral, antimicrobic, radio protective and immune action. The problem of clinical application of interferons is actual for a wide range of experts, including clinicians, and also doctors of practical public health services. Increasing interest of doctors of different specialties to this problem is caused not only high efficiency of application of preparations, but also steady growth of number of the patients, suffering by virus, infectious, oncologic diseases.

Abstract

The arterial hypertension complicated by development of a brain stroke is one of the main reasons of population disability. In the article there are presented features of circadian blood pressure profile and arterial stiffness at patients with an ischemic stroke with uncorrected arterial hypertension. It is established that the increased variability of arterial pressure, pulse wave velocity and central aortal pressure are starting factors of ischemic stroke development and these parameters may be used as predictive indicators of a functional outcome of acute ischemic stroke.

Abstract

Low heart rate variabilit y (HRV) is associated with increased cardiovascular mortality. 108 patients with acute ischemic stroke are surveyed. It is established that there is a decrease of HRV and circadian index (СI) under moderately severe and severe strokes. The reduction of HRV is connected with age, hyperglycemia, and abdominal obesity. Significant influence of stroke’s center localization on HRV is not revealed. The decrement of HRV indices is revealed after 10 days under mild and moderately severe strokes. As for heavy stroke- the indices remain low. СI had positive dynamics.

Abstract

Various disorders of the diastolic function of left ventricle are formed due to concentric and eccentric types of hypertrophy. The processes of elevation of myocardial stiffness and reduction of lard-bore arterial vessel elastance develop parallel and at the early stages are accompanied with the rise of isolated systolic hypertension frequency, at the later stages — isolated systolic and systolic-diastolic arterial hypertension.

Laboratory biomarkers and predictors of use for risk assessment in clinical practice and disease prevention. Clinical laboratory diagnostics as a branch of medical science and the element of the health care system evolves, and this requires constant updating of knowledge in the discipline.

Abstract

Of 125 patients included in the study 70 had rheumatoid arthritis (RA). They were compared to 40 patients with deforming osteoarthritis (DOA) and 15 patients with atherosclerosis of great cerebral arteries (MCA). Control group consisted of 40 persons having remission randomized according to age and sex. Carotid, vertebral arteries and cerebral circulation were investigated with ultrasound duplex scanning on the unit «Vingmed system 5» (Norway, 2002). Duplex scanning in RA patients in comparison with control group and OA patients showed the increase of the stenotic lesions of the carotid and the vertebral arteries which is more evident in group by seropositive RA and in RA patients with extra-articular manifestations. Significant data were received when studying the carotid deformation. Comparison of the RA groups showed considerable differences: in RA group with systemic manifestations increase of the sinuation rate became evident both in common carotid arteries (χ2 = 1,76; NA), and in internal carotid arteries (χ2 = 8,44; p = 0,01). Data received in present study indicate that in RA patients changes of cardiovascular system, characterized by forwardness of the atherosclerosis, take place. Evidence of the atherosclerotic changes associates with presence of RA systemic manifestations, high inflammatory process activity and seropositivity by rheumatoid factor.

Abstract

The condition of bone mineral density (BMD) in 122 patients with joint hypermobility syndrome (JHS) had been studied. The musculoskeletal and articular manifestations were defined; the range of orthopedic pathology was clinically and radiologically verified. The prevalence of osteopenia with detalisation of the degree of bone density reduction was determined by osteodensitometry. The timely detection of low bone mass with the modification of risk factors is reasonable in preventive strategy in patients with JHS.

Abstract

Until recently low-energy fractures in the elderly associated primarily with osteoporosis, not paying enough attention to the prevention of falls. The rationale for the use of combination therapy consisting of treatment of osteoporosis (bisphosphonates) and prevention of falls (alfacalcidol, calcitriol, non-drug methods) in order to prevent fractures in the elderly.

Abstract

The aim of this work is investigation of H. pylori eradication on heart structural and geometry parameters and myocardial function at patients with coronary artery disease (CAD) and gastroenterological pathology. We studied 147 patients with CAD and gastroenterological pathology. It was stated, that H. pylori infection causes negative effect on intracardial hemodynamics and myocardial function at patients with combined pathology. We also fixed, that helicobaceriosis led to more clear myocardium remodeling and finally to cardiac insufficiency. It was shown, that eradication therapy optimized CAD symptoms by intracardial hemodynamics normalization.

Abstract

The аrticle is devoted to the first head of the chair of infectious diseases of RGMI professor B.N. Stradomsky. In addition autobiographical data chronologically describes the way of becoming a great scientist and educator from a student, intern, the clinician to, head of chair of infectious diseases of Rostov state medical institute. Basic research B.N. Stradomsky were devoted to the diagnosis and treatment of malaria. Accumulated vast practical experience in malaria foci formed the basis of the thesis and monograph «To the question of the treatment of malaria». The article also shows the relevance of the conclusions B.N. Stradomsky malaria and at the present stage.
