According to big clinical trails Mg 2+ deficiency plays a major role in health and promote development of different chronic diseases. It is necessary to cooperate clinical symptoms and results of objective methods (ECG, myography, bone densitometry, level of Mg 2+ in hole blood, erythrocytes, serum, saline, urine, nails, hair) in case of diagnosis Mg 2+ deficiency. It is desirable to analyze Mg2+ intake with food by standard questionnaires. To have the full picture of Mg 2+ deficiency it is necessary to define level of Mg2+ in different biosubstrates.
The optical parameters of red blood cells and serum from patients with different classes of liver cirrhosis by Child–Pugh were examined by atomic force microscopy, dielectrophoresis, spectral ellipsometry, terahertz and Raman-spectroscopy. We found that with the growth of the class of cirrhosis the level of the deformed shapes of cells with altered membrane relief was increased and transmission power terahertz suspension of erythrocytes was reduced. Among the viscoelastic and electrical parameters of erythrocytes some characteristics (summary measures of viscosity, the relative polarizability, electrical conductivity, the capacity of the cell membrane, the position of the crossover frequency) were the most important for determining the severity of liver cirrhosis. The growth of decompensation of cirrhosis resulted in lower the thickness of thin films prepared from the serum, with the rise of the refractive index and heterogeneous structure. At the same time, we observed a significant decrease of the peak areas of the absorption Raman- spectra at 1005, 1157 and 1520 cm-1.

To evaluate the prognostic significance of markers of fibrosis in the risk of transition persistent atrial fibrillation in chronic form. It was revealed that the performance of remodeling of the left atrium (the level of PICP, TIMP-1, and the index of the PL to the BSA) have predictive value in the risk of progression of AF. The risk of transition to a chronic form of arrhythmia increases as the concentration of PICP over 133 ng/ml (Se 70–73% and Sp 62–75%), at a concentration of TIMP-1 less than 490 ng/ml (Se 67–78%, Sp 69–75%) and index of LP to PPT than 32–34 ml/m 2 (Se 80%, Sp 77–83%). For patients with AF against hypertension risk relapsing forms of transition to permanent AF occurs when the level of PICP over 151 ng/ml (1.01 fold increases in each of the subsequent increase in the concentration of 1 ng/ml) and the value of the index volume ma LP PPT to over 38 ml/m 2 (by 1.5 times for each subsequent increase of 1 ml/m2).
In patients with symptoms of stable angina coronary angiography often fails to reveal significant atherosclerosis. So far as prognosis in these patients is better than in patients with significant coronary atherosclerosis (SCA) the factors associated with absence of SCA in elderly patients with stable angina have to be determined. This is a retrospective study of consecutive 16.839 patients who underwent coronary angiography. We selected 2.911 patients with stable angina and without SCA: 415 patients ≥ 65 years old and 2.496 patients younger than 65 years. Hypothyroidism as a predictor of the SCA absence in elderly patients with stable angina had the higher coefficient of equation of linear regression (OR = 1.75; 95% CI 1.13–2.71; p = 0.013). Authors hypothesized that hypothyroidism can be the cause of microvascular myocardial dysfunction which manifest clinically as stable angina in elderly patients with the absence of SCA.

The problem of pain in the glenohumeral region is an urgent problem for many decades. The article describes the clinical aspects of diagnosis and treatment of pain in the glenohumeral region, particularly in the application of conservative treatment of pain along with nonsteroidal anti-inflammatory drugs neurotropic complexes Milgamma.
The article is devoted to a new direction in medical treatment of ischemic heart disease and arterial hypertension — the use of cytoprotective and antioxidant agents. The intracellular mechanisms of development of hypoxia and ischemia, fabrics, oxidative stress, possible ways of correction of these processes are discussed. Classification of cytoprotectors considered their mechanisms of action, indications, results of their application in various forms of ischemic heart disease and arterial hypertension, and other conditions involving oxidative stress and vascular dysfunction of endothelium are given.
ОБЩЕЕ ДЕЛО
Objective of the study is to analyze the dynamics of blood pressure in the early postoperative period in patients with a tumor of the kidney and hypertension after laparoscopic nephrectomy. Group for laparoscopic nephrectomy selected 53 patients with hypertension aged 37–65 years. Duration hypertension patients with kidney tumors was 6.5 ± 3.2 years. All patients to assess the effectiveness of the primary purpose or need antihypertensive therapy before the surgery was conducted ambulatory blood pressure monitoring, and with the purpose of correction of antihypertensive therapy after surgery blood pressure monitoring performed on the second and tenth day using the monitor МECG-DS-HC-01. Strengthening of antihypertensive therapy took all the patients on the second day after the operation, on the tenth day correction of antihypertensive therapy was needed 32 patients (60.4%). Once added to the treatment scheme nifedipineretard the decline of some indicators of blood pressure monitoring on the tenth day of the postoperative period. Further reception of antihypertensive drugs after surgery allowed to achieve the target values of blood pressure.
In recent years, more attention is paid to chronic heart failure with preserved ejection fraction. This report examines not only the pathogenesis of diastolic heart failure, but the features of the pathogenesis of chronic heart failure with preserved ejection fraction, developing in patients with comorbidity for type 2 diabetes. The article also presents data as existing and prospective methods of diagnosis of these pathologies.
ПСИХОСОМАТИЧЕСКАЯ МЕДИЦИНА
The prevalence of psychopathological syndromes in patients with chronic obstructive pulmonary disease (COPD) is estimated in this article. The impact of the disease on comorbid psychiatric disorders severity and impact of their treatment on the COPD symptoms are considered. 5 most frequent psychiatric syndromes were identified. The efficacy against mental disorders and symptoms of underlying disease for individual pathogenetic psychotherapy and therapy in the group for improving the quality of life in patients with COPD is shown.

The article contains a description of a clinical case of refractory hypotension, developed after surgery for pituitary macroadenoma. Postoperative period was complicated by the development of acute myocardial infarction, connected with aggravation of megakariocyte leukemia. Presented differential diagnosis causes hypotension in a patient with severe comorbidity.

Smoking is widespread among young people and is a main risk factor for chronic obstructive pulmonary disease. In this study we investigated the prevalence of smoking among medical students, its effect on lung function parameters and serum levels of soluble forms of adhesion molecules (CD50, CD54). We founded a reduction in FVC, PEF and MEF 25, and increased serum level of sCD54 in smoking students, indicating the early signs of airway obstruction and dysregulation in the system of adhesion molecules in these persons.

German doctors, both who were educated abroad and those who obtained a diploma in Russia, played a great role in a development of Russian health care and science. Authors describe the history of the first German doctors’ arriving in Russia, introduce stages of a new tradition establishment – inviting foreigners to work as royal doctors, mention a role of Germans in a professional medical societies’ foundation. Authors show a great volume of a professional and social work of Russian doctors, Germans by birth, country regions, taking Nizhny Novgorod region as an example. Besides, there is said what relations did German doctors have with aborigines.
