Abstract

Hypermobility of joints (HM) and closely associated with him Joint Hypermobility Syndrome (JHS) are very distributed in clinical practice. The family nature of the HM had been found that suggests a genetic nature of the observed phenomena. This syndrome has got a dominant inheritance, mainly through the female line. At the present time we’ve got a sufficient information wich shows HM influences on the occurrence and course of the artrologic diseases. JHS is a factor of risk for the development of early osteoarthritis (OA) which arises in these individuals more often than the other people. Some foreign scientists associate HM and OA, another one suppose that HM provides a protective effect on OA. The issue of the relationship between HM and early OA is discused. Persons with HM, especially if in their families there are patients with OA, it is necessary to recommend preventive measures.

Abstract

The IHD name doesn’t completely reflect the disease essence. Its second name «coronary heart disease», that is widely spread in the west medicine, is even wronger. Systemic pathologists, cardiologists, that were one of the originators and their followers were mislead by the eye-catching coronary occlusion and the base method of the disease investigations, that was used then and used now — the method of the clinico-anatomical apposition (CAA). The described problem brought to the point that all the efforts taken by the scientists to the every possible heart circulation management didn’t succeed. IHD incidence and mortality stay very high and in most of the countries keep increasing. System analysis, used instead of CAA gave an opportunity (according to this article’s author) to find the real reason and this disease mechanism and to create a new system concept of the IHD matter. That will allow to change the approach to the disease and its cure cardinally.

Abstract

In article authors discuss invasive strategy of treatment of patients with acute coronary syndrome without ST elevation. 221 cases from CCU of hospital № 7 were analysed. There are some problems in pre-clinical diagnosis (half of cases were excluded diagnosis of acute coronary syndrome write after hospitalisation) and hight specifity of preliminary clinical diagnosis, that was approved in 82% of cases. Only 26% of patients with acute coronary syndrome without ST elevation were refered to surgery (in 15 cases — percutaneous coronary intervention, in 3 — coronary artery bypass grafting). After analisation of medical histories authors find out patients to whom surgery was needed but wasn’t prescribed (in 29 cases — contrindications, in 23 — recomendation to deferred coronaroangiography and 15 were discharged without stress test performance. In situation we will use new diagnostical tools and methods we can find more cases of acute coronary syndrome without ST elevation.

Abstract

In article we provide clinical case of patient with high level of creatine phosphokinase. For specialists in internal diseases high level of creatine phosphokinase means fi rst of all differential diagnosis between: acute myocardial infarction, hypothyroidism and demiatomyositis as a separate disease or as a part of paraneoplastic syndrome. But, there are some more conditions that can lead to high level of creatine phosphokinase. We provide our case to pay attention on necessarily of accurate collection of anamnestic points, examination of patient and analysis of laboratory and instrumental examination.

We provide description of clinical case of chronic abdominal ischemia, which is usually associated with great variety of symptoms and low specificity. Such patients spend a lot of time for different specialists and different types of laboratory and instrumental analysis. Abstract

Osteogenesis imperfecta syndrome characterized by clinical manifestations of polymorphism under light for a long time can not be diagnosed, and it becomes apparent only after the symptoms found in children and grandchildren these patients. Management of patients with this nosology requires a multi-component approach in the context of the family with questions of prevention of recurrent fractures, treatment, rehabilitation and disability.
