Abstract

Today secondary prevention of coronary heart disease (CHD) is one of the main strategic objectives of the Russian and world healthcare systems. Increasing importance is attached to the use of drugs, optimizing energy exchange of myocardial cells, such as coenzyme Q10 (Qudesan). This article presents the results of research of the effectiveness and tolerability of Qudesan in patients with CHD.

Abstract

The article reviews mechanisms of insulin resistance, disorders of carbohydrate and lipid metabolism in patients with metabolic syndrome. Based on literature data and expert opinions of the World Health Organization the epidemiology and prognosis of the number of patients with diabetes mellitus (DM) type 2 in 2025 were presented. Based on the experience of examination and treatment of 75 patients with metabolic syndrome and impaired glucose metabolism from prediabetes to DM type 2 the predictive role of daily monitoring of glucose in the efficiency of the complex (Metformin, Tricor 145, Actovegin) in patients with this disease were shown.

Abstract

We have studied the clinical efficacy of Sulodexide in patients with diabetes mellitus type 2 and coronary heart disease in the context of prevention of contrast-induced nephropathy (CIN). Inclusion of Sulodexide in the complex therapy of patients with coronary artery disease and type 2 diabetes requiring intervention measures creates the preconditions for reducing the incidence of CIN and prevention of kidney function, providing antiproteinuric effect, correcting lipid metabolism, blood coagulation system.

Abstract

In this report we present current data on prevalence, clinical significance and methods of diagnostics of occult hepatitis B. Mechanisms of occult hepatitis B formation are reviewed, among which the most important is co-infection with other hepatotropic viruses. Diagnostic significance of anti-HBcAg antibodies is shown. An algorithm of occult hepatitis B detection developed by the authors involves two stages: detection of viral markers in serum and liver tissue. Attention is drawn to the role of morphologic methods including histochemistry, immunohistochemistry and electron microscopy in verification of HBVinfection and direct visualization of virions in cytoplasm of hepatocytes.

Abstract

An assessment of the possibility of optical methods for studying red blood cells and serum of patients with diffuse liver disease with varying degrees of fibrosis for use in early diagnosis has been made. It is shown that the use of combined optical methods can significantly improve the performance of sensitivity, specificity, accuracy index and achieve real results in the diagnosis of both pronounced fibrosis and slight ulterior liver fibrosis. Identified opportunities of optical methods may be promising in screening non-invasive staging in diffuse liver disease of various origins.

Abstract

Testing for carriers of mutations C282Y and H63D HFE gene in 57 patients with nonalcoholic fatty liver disease was completed. Abnormalities in the metabolism of porphyrins were detected in 39 (68.4%) patients, mutations C282Y and H63D were detected in 16 (28.1%) patients, of whom 12 patients with metabolic disorders of porphyrins and symptoms of the syndrome of chronic iron overload. In 41 (71.9%) patients without the mutations found disorders metabolism of porphyrins were in 27 (65.8%) patients. They had no symptoms of the syndrome of chronic iron overload. Detection of C282Y and H63D mutations in the gene HFE in conjunction with disorders of porphyrin metabolism in association with the syndrome of chronic iron overload, but the probability will consider these patients as candidates for inclusion in the higher risk of formation of liver fibrosis.

Abstract

The analysis of functional activity of platelets at patients with middle severity of illness of Astrakhan rickettsial fever with accompanying hypertensive illness depending on age was carried out. At patients were more senior 50 years of change of activity platelets in an illness heat were expressed more brightly and weren’t restored during the period of reconvalescence.

Abstract

Arterial hypertension (AH) is associated with hypothyroidism, diffuse lesion accelerates the progression of the vascular bed of the great arteries to the capillaries. Objective was to assess the condition of the microcirculation in patients with AH associated with metabolic syndrome (MS) and subclinical hypothyroidism (SH). The study included 75 patients divided into 3 groups: 23 patients with AH, 29 patients with AH in the MS, 23 patients with AH associated with MS and SH. All the patients underwent examination of the state of microcirculation by laser doppler flowmetry. It is established that the presence of even the initial thyroid hypofunction in patients with AH in the MS leads to a decrease in the ability of the physiological modulation of blood flow in the microvasculature.
